We all know when a thief can enter in our house. When we leave our doors and windows open or don't lock our home. In a similar way hackers are the thief's in computer world which attack when they see any open way. Now How do you know Your computer lock is open or you don't lock the ports by which they attack.
They are looking for a open way to enter in your computer via internet connection. If they find an port open in your computer's internet connection they launch an attack on your system so that they can access your network.

The tools they use can be preloaded with a range of Internet addresses to scan. As this tool finds an Internet address with certain openings it produces a list of the address and the opening.
This list is then fed into another tool that actively tries to exploit that opening with various programs. If no exploit works, the hacker's program moves on to the next potential victim.
